Overview

The Baxter Mini-Infuser 150XL is a compact, lightweight, battery-powered linear syringe pump designed for the controlled intermittent administration of intravenous medications. The pump was originally developed by the Bard Medsystems Division of C.R. Bard, Inc. and first cleared by the FDA in 1993 under the names Bard Mini-Infuser 1500 and 3000; it was later marketed by Baxter Healthcare Corporation as the Mini-Infuser 150XL (single-rate) and Mini-Infuser 300XL (multi-speed). The 150XL holds a standard 5 mL, 10 mL, 20 mL, 30 mL, 35 mL, or 60 mL disposable syringe in a barrel clamp, advances the plunger at a fixed linear rate of 5 inches per hour, and delivers fluid through a Microbore extension set into a heparin lock or piggybacked into a primary IV line. Because the 150XL drives the plunger at a single fixed rate, the resulting fluid flow rate is determined by the diameter of the syringe selected, allowing the clinician to choose a drug concentration and syringe size that produce the desired delivery time. Its small footprint, durable ABS case, and battery-only operation make it useful for short-duration medication infusions in critical care, oncology, infectious disease, neonatal, pediatric, and ambulatory care settings.

Display, Indicators, and User Interface

The Mini-Infuser 150XL uses a simplified mechanical interface rather than a programmable display, which is part of why it remains popular for facilities that need a reliable single-purpose device with minimal training overhead. Operation centers on a three-position switch (Off, On, and On with Alarm) and a release lever that disengages the pusher and anti-syphon latch for syringe loading and unloading. Three colored LED indicators communicate pump status: a flashing green light confirms normal operation, a flashing red light signals end-of-syringe or an occlusion, and a flashing yellow light indicates a low-battery condition. In the On with Alarm position, an audible beeping alarm mimics the flashing red light to provide both visual and acoustic notification when the syringe empties or when downstream pressure exceeds the occlusion threshold. The On position provides visual alarming only, which is appropriate where ambient noise sensitivity matters. A loading time scale and indicating mark on the case allow the clinician to estimate the remaining infusion time at a glance based on the syringe size in use.

Drug Delivery and Infusion Configurations

The Mini-Infuser System is designed around three primary administration configurations described in the Baxter operator’s manual. The first configuration uses the pump as an intermittent infusion system in conjunction with a heparin lock, with a Microbore extension set connecting the syringe to the patient access. The second configuration is a piggyback delivery in combination with a gravity primary IV line, where the Microbore extension set is attached to the lower injection port or flashball site of the primary set so that the primary flow continues uninterrupted while the syringe contents are delivered. The third configuration is a piggyback in combination with a volume-control chamber such as a Buretrol set, which allows further dilution of the syringe contents into the burette before delivery. Because the 150XL drives the syringe plunger at a constant 5 inches per hour, the infusion duration depends on the syringe size: smaller syringes deliver smaller total volumes faster, while larger syringes deliver greater volumes over longer periods. The operator’s manual provides nomograms for each compatible B-D and Monoject syringe so clinicians can quickly determine fill volume and total minutes-to-complete for any planned dose. Drugs are typically diluted to a final concentration in the syringe such that one syringe delivery equals one prescribed dose at the desired duration.

Single-Rate Mechanical Design and 150XL Versus 300XL

The Mini-Infuser product family includes two pumps that share the same chassis, syringe holder, and electrical platform but differ in their delivery flexibility. The 150XL is a single-rate pump that drives the plunger at a fixed 5 inches per hour and is selected when a facility wants the simplest possible workflow for intermittent piggyback infusions. The 300XL is a multi-speed version with a rate selector switch on the back of the case that allows the user to choose among multiple plunger drive speeds, expanding the practical infusion duration range without changing syringe sizes. Because the 150XL is a fixed-rate mechanical pump, it does not maintain a programmable drug library, dose error reduction software, or any electronic settings beyond the on/off/alarm switch. The same battery, syringe holder, anti-syphon latch, and alarm logic are shared between the two models, which simplifies parts inventory and biomed training when a facility carries both. Battery Operation, Mounting, and Portability

The Mini-Infuser 150XL is exclusively battery operated and does not include an AC power option. Four standard “C” size alkaline cells provide approximately 400 hours of typical service life, and the pump alarms with a flashing yellow light at a battery threshold of 4.0 ± 0.2 volts; once the low-battery indication appears, approximately 4 hours of remaining infusion time is typical before the pump can no longer drive the plunger. Battery selection is operationally significant because the FDA-classified Class 3 correction in 2011 (described below) traced inoperability to a contact-diameter incompatibility between the pump’s battery spring and certain over-the-counter C cells, so consistent sourcing of compatible alkaline cells matters for reliability. The pump weighs 1.75 lbs with batteries installed and measures 5 inches tall by 4.5 inches wide by 1.6 inches deep, which makes it convenient for ambulatory and bedside use. Mounting options include an integrated IV pole loop molded into the case and an optional mounting bracket that secures the pump to bedrails, headboards, transport carts, and other surfaces. The impact-resistant ABS molded enclosure is appropriate for routine clinical handling, though the manual notes the pump should not be immersed and is not suitable for use in the presence of flammable anesthetics.

Service, Maintenance, and Calibration Considerations

Baxter has ended factory service on the Mini-Infuser 150XL, which means hospitals that continue to operate these pumps must rely on third-party service organizations or in-house biomedical engineering teams for repair, calibration, and parts. The Baxter operator’s manual specifies a regular calibration check that includes verifying free movement of the syringe holder mechanism, confirming proper retention by the anti-syphon latch, validating the audible alarm, and confirming that downward force at the syringe holder of approximately 5.0 ± 1.0 lbs triggers the occlusion alarm within the specified 70-second detection window. The manual also recommends lubricating the lead screw at least every six months using the manufacturer-supplied grease and applicator. Because the 150XL has no electronic display, no event log, and no networked diagnostic interface, biomed inspections rely on direct mechanical and electrical checks rather than on software-based service screens. FDA Device Corrections and Safety Bulletins

The FDA Medical Device Recalls Database lists one device correction for the Mini-Infuser 150XL: a Class 3 recall, FDA recall number Z-2268-2012, initiated by Baxter Healthcare Corporation on March 4, 2011 and terminated on August 27, 2012. The correction addresses a battery contact compatibility issue. The Mini-Infuser battery spring has a flat contact measuring approximately 0.53 inches on the inner diameter of the spring coil; the IEC 60086-2 and ANSI 08 standards specify a minimum contact diameter for C-cell batteries of 0.51 inches. If a C cell with the minimum allowable contact diameter is installed, the contact circuit may not complete and the Mini-Infuser will not power on. The same battery interface design is shared with the Baxter InfusO.R. pump, and Baxter expanded the original InfusO.R. safety alert (issued January 11, 2011) to include the Mini-Infuser 150XL and 300XL with the March 2011 correction. The recall covers all serial numbers of product codes 2M8170 (new) and 2M8170R (refurbished). Approximately 26,485 units were affected and the distribution pattern covered roughly 45,865 units. The FDA classified the action as Class 3, indicating that use of the affected product is not likely to cause adverse health consequences, because the failure mode is a clear and immediate failure to power on rather than a delivery accuracy or alarm failure during active infusion.
