Medfusion 4000
**THE SPRING SALE – Includes the latest software upgrade resolving prior FDA concerns. Sold without accessories. Cosmetic condition may vary. All units are fully tested and quality assured before shipment.**
The Medfusion 4000 is a wireless smart syringe infusion pump trusted by top pediatric, specialty, and adult hospitals for accurate, safe medication delivery in the NICU, PICU, adult critical care units, and the operating room. Built on the same proven platform as the Medfusion 3500 and recognized for over a decade of clinical use, the 4000 advances that foundation with wireless connectivity, an expanded drug library, FlowSentry pressure monitoring technology, and PharmGuard Medication Safety Software — all in a horizontal design that allows single-handed syringe loading.
It delivers from 0.01 mL/hr to 1,130 mL/hr at nominal ±2% accuracy across syringes from 1 mL to 60 mL, with delivery modes spanning continuous, volume/time, mass, body weight, intermittent, and bolus. Wireless or wired connectivity to the PharmGuard Server enables remote drug library updates, firmware deployment, and continuous quality improvement reporting without physically handling each pump.
Smiths Medical issued an FDA Class I device correction covering Medfusion 4000 pumps running software versions v1.0.0, v1.1.0, v1.1.1, and v1.1.2, which were found to have issues including alarm failures, re-administered loading doses, and improper delivery behaviors. The correction required all facilities to confirm their pumps were updated to the most recent software version. IPR Medical can perform software updates on your Medfusion 4000 pumps to resolve all issues identified in the FDA device correction, bringing your fleet into full compliance. Whether you need a patient-ready unit, replacement parts, a software update, or a repair, IPR Medical is here to help.

The next generation of the trusted Medfusion platform
The Medfusion 4000 is built on the same proven pump platform as the Medfusion 3500 — already widely recognized for accurate medication delivery across critical care, NICU, PICU, and the OR. The 4000 advances that foundation with wireless connectivity, an expanded on-board drug library of over 4,600 protocols, and full wireless PharmGuard Server integration for fleet-wide drug library and firmware updates without physically handling each pump. The core pump mechanics, FlowSentry pressure monitoring, ±2% accuracy, syringe compatibility range, and delivery modes are shared with the 3500 platform. If your facility does not require wireless connectivity, the Medfusion 3500 Version 6 is available here.
PharmGuard and wireless fleet management
The Medfusion 4000 connects wirelessly or via Ethernet to the PharmGuard Server, enabling remote deployment of drug library updates and firmware without physically handling each pump in the fleet. PharmGuard Medication Safety Software supports hard and soft dosing limits to catch programming errors before they reach the patient, and provides CQI reporting for pharmacy, nursing, biomedical engineering, and medication safety officers. The Quick Library feature allows certain infusions to be started in as few as three steps — Profile, Drug Selection, Start Infusion — reducing setup time in high-acuity environments.
FlowSentry pressure monitoring and occlusion detection
FlowSentry is a continuous pressure sensing technology built into the Medfusion 4000 that monitors delivery pressure and analyzes changes for earlier clinical intervention — before a full occlusion alarm fires. Four occlusion sensitivity levels (Very Low ~4 psi, Low ~8 psi, Normal ~12 psi, High ~16 psi) are selectable and can be overridden per infusion via the Options menu. FlowSentry can also be disabled for individual infusions when clinical conditions require it.
FDA device correction — software update required
Smiths Medical issued an FDA Class I device correction covering Medfusion 4000 pumps running software versions v1.0.0, v1.1.0, v1.1.1, and v1.1.2. The correction identified issues including a high-priority alarm failure during motor-not-running conditions, re-administration of loading doses, and other delivery and alarm behaviors that could lead to over- or under-delivery of medication — particularly in NICU and CICU patients. The correction required all facilities to confirm their pumps were updated to the most recent software version.
IPR Medical can update your Medfusion 4000 pumps to the current software version, resolving all issues identified in the FDA Class I device correction and bringing your fleet into full compliance. Contact us to arrange software updates alongside any sales, repairs, or parts needs.
