Overview
The Smiths Medical Medfusion 4000 is a small, lightweight, portable syringe infusion pump designed for precisely controlled low-volume delivery in the most demanding care areas, including neonatal and pediatric intensive care, adult critical care, anesthesia, and the operating room. It delivers fluids requiring tightly controlled infusion rates — blood and blood products, lipids, drugs, antibiotics, enteral solutions, and other therapeutic fluids — from a wide range of syringe brands and sizes spanning 1 mL to 60 mL, with automatic syringe size detection. The pump supports arterial, epidural, intravenous, intrathecal, subcutaneous, and enteral routes. Built on the same trusted drive platform as the Medfusion 3500, the 4000 pairs PharmGuard® Medication Safety Software with FlowSentry™ rapid occlusion detection so that the pump is in a medication-safety state from the moment it powers up, helping facilities enforce their own dosing policies at the point of care.
Display and User Interface
The Medfusion 4000 uses a backlit color display paired with a hardware keypad and context-sensitive softkeys. During an infusion, the softkeys give direct access to the main menu, dose changes, the options menu, and clearing of stored infusion volume and dose totals, so clinicians can adjust therapy without navigating away from the active delivery screen. Pressure trending is shown directly on the display, allowing the clinician to watch occlusion pressure climb and intervene before an alarm condition is reached. The interface presentation is configuration-driven: the modes available on a given pump, and where they appear on screen, are defined by the institution’s PharmGuard® Toolbox 2 configuration rather than fixed at the factory. This lets a biomed or pharmacy team standardize the look and behavior of every pump in a fleet to match unit-specific workflows, which shortens training time and reduces the chance of programming error. A battery indicator and audible alarm-silence controls are available directly from the keypad.
Drug Library and Dose Error Reduction
PharmGuard® Medication Safety Software is the core of the Medfusion 4000’s dose error reduction system. Libraries are organized by Profile, and each Profile supports up to eight library Categories of up to 36 Drug Programs each — a total of 288 entries per Profile — plus a single Quick Library per Profile for rapid setup of the most common drugs. Profiles let a facility tailor care-area-specific drug protocols, so a NICU pump and an adult critical care pump can enforce different concentration and dose limits from the same standardized configuration. Drug Programs can carry hard and soft limits that alert the clinician when a programmed value falls outside expected ranges, and limit events are recorded for continuous quality improvement review through PharmGuard® Server reporting. The pump supports an extensive range of continuous and intermittent delivery modes: dose per minute/hour/day, dose per kg per minute/hour/day, dose per m² per minute/hour/day, mL/hr, mL per kg per minute/hour, dose per kg per time, dose per m² per time, mL per kg per time, volume/time, dose/time, and intermittent volume/time. Delivery options include volume limit, bolus dose, loading dose, KVO, standby, periodic callback, delayed start, and FlowSentry™ toggling.
Software Versions and Platform Evolution
The Medfusion 4000 has shipped in multiple firmware versions over its service life, and the installed version is shown on the pump’s start-up screen. The version is operationally important: drug-library compatibility, available features, and the applicability of FDA corrections all depend on it, so the version should always be confirmed against the matching Operator’s Manual before a pump is placed in service. Early production ran on the 1.x firmware series (including v1.0.0 and v1.1.x, the version family covered by the original Operator’s Manual). Later 1.x releases referenced in subsequent OEM and FDA documentation include the v1.5.x and v1.6.x families and v1.7.0. Smiths Medical’s 2024 field correction identifies v6.0.0 as the remediated software version that resolves the issues described in that action, indicating that facilities running earlier firmware should plan for the software update. Because exact version histories and update eligibility vary by individual pump and by the PharmGuard® Server environment in use, IPR Medical confirms the installed firmware version on every unit it sells or services and can advise on the correct update path for your fleet. Contact IPR Medical to verify the version on a specific pump.
Battery, Mounting, and Portability
The Medfusion 4000 is designed for continuous bedside use and for transport between care areas. It runs on a rechargeable lithium-ion battery rated for a minimum of 400 charge cycles, providing approximately 10 hours of typical runtime at 5.0 mL/hr with a 60 mL syringe from a full charge. On AC power the pump accepts 100–240 VAC at 50/60 Hz and is rated Safety Class II, Type CF, making it suitable for direct cardiac application and for use on international mains without a converter. The pump weighs approximately 4.54 lbs and measures 10.5 inches wide by 6.2 inches high by 5.7 inches deep, with an IPX3 sprayproof rating for resistance to incidental fluid exposure. A rotating poleclamp accessory allows flexible orientation on an IV pole, which is useful when several pumps are stacked on a single channel for a complex multi-drug patient. The pump is also MR Conditional, cleared for use inside the MRI room when mounted outside the 150 gauss line with shielded magnets of field strength up to 1.5 Tesla — a meaningful capability for sedation and contrast workflows in imaging suites.
Wireless Drug Library Updates and EMR Integration
The Medfusion 4000 imports and exports data to support history management and fleet-wide drug-library governance. The pump communicates bi-directionally with the PharmGuard® Server over both wireless (IEEE 802.11, 2.4 GHz) and wired Ethernet connections on the institution’s network. Through this link, configurations and firmware can be pushed to pumps, and infusion settings and event history can be downloaded, in combination with the PharmGuard® Toolbox 2 software application. Wireless connectivity is what enables a pharmacy or biomed team to distribute an updated drug library across an entire fleet without physically handling each pump, and to close the continuous-quality-improvement loop by pulling event and alarm data into PharmGuard® Server reports. Configurations are created on a PC in PharmGuard® Toolbox 2 — including global settings, Profile definitions, library and Drug Program definitions, and syringe definitions — and then sent to the pump. Facilities integrating pump data into electronic health record and infusion-documentation systems should validate the specific server software version and network configuration against their environment, as message handling between the pump, the PharmGuard® Server, and downstream EHR systems has been the subject of FDA-tracked corrections; see the FDA Device Corrections section below.
Service and Maintenance Considerations
Like all syringe infusion pumps, the Medfusion 4000 benefits from a defined preventive maintenance program covering battery condition, occlusion-sensor calibration, keypad function, and firmware currency. The pump runs continuous hardware and software self-tests — a dedicated watchdog circuit monitors the main processor, and power-on and background checks verify program memory (via CRC), RAM, motor operation, the keyboard encoder, EEPROM-stored critical data, and the real-time clock — halting delivery and alarming if a fault is detected. Because the occlusion force sensor and the syringe drive are central to delivery accuracy, calibration and mechanical wear are the items most worth tracking over a unit’s life; both have been the subject of OEM field corrections (force-sensor calibration drift and a barrel-clamp guide molding issue), which makes documented service history valuable when evaluating a used pump. IPR Medical inspects and tests each Medfusion 4000 it handles, confirms the installed firmware version, and can supply replacement batteries, poleclamps, administration sets, and supported syringes. For full disassembly, calibration, and replacement-part procedures, the OEM publishes a dedicated Medfusion 4000 Series Technical Service Manual; contact IPR Medical for service support or to arrange a repair.
FDA Device Corrections and Safety Bulletins
The Medfusion 4000 has been the subject of several FDA-tracked field corrections over its service life. The most significant current action is a Class I correction initiated by Smiths Medical on December 19, 2023 (FDA recall number Z-0864-2024), issued to customers as an Urgent Medical Device Correction. It addresses several software-related issues on affected Model 4000 firmware versions, including delivery during a Motor Not Running high-priority alarm, a re-administered loading dose, an incorrect critical-data-failure alarm, and interruption of bolus or loading dose delivery, among others. Smiths Medical identified the root cause as a software design issue and points to software version v6.0.0 as the remediation. A separate earlier Class I correction from 2020 (FDA recall number Z-2734-2020) addresses inaccurate delivery that can occur following an interrupted bolus or loading dose if a specific sequence of events occurs, affecting firmware versions in the v1.5.x and v1.6.x families. An earlier Class II correction from 2013–2014 (FDA recall number Z-0540-2014, since terminated) addressed electrical shorting at the AC power receptacle when conductive fluids entered the receptacle. Facilities should confirm the firmware version installed on any Medfusion 4000 in their fleet and determine whether the corresponding software update or corrective action applies.
Because FDA device corrections and safety communications may be updated or expanded after the date of this listing, IPR Medical recommends that facilities also check the FDA’s official resources directly to ensure they have the most current information.
