Overview

The GE CARESCAPE B650 is a modular, multi-parameter patient monitor built for use across a broad range of acuity settings within a professional healthcare facility, including critical care, post-anesthesia, emergency department, neonatal, and lower-acuity operating room environments. It is designed as part of the CARESCAPE monitoring platform, which standardizes algorithms, parameters, and user interface behavior across bedside monitors, central stations, and transport devices. The B650 consists of a main processing unit, a 15-inch display (available in touchscreen and non-touchscreen versions), and a pivoting module frame that accepts the Patient Data Module (PDM), Patient Side Module, and CARESCAPE E-series plug-in modules. This modular architecture allows a single bed to transition between measurement configurations without swapping monitors, which supports consistent data capture as patients move between units. The B650 is intended for monitoring adult, pediatric, and neonatal patients and supports intrahospital transport when equipped with the optional internal lithium-ion battery.

Display and User Interface

The B650 uses a 15-inch color LCD as its primary display and control surface, available in touchscreen and non-touchscreen versions. Clinicians can navigate menus directly on the screen or through an integrated Trim Knob control, external USB keyboard, mouse, barcode reader, or remote control and keypad. The main screen presents waveforms, parameter windows, trends, alarm status, and the main menu, with on-screen selections confirmed through touch or the Trim Knob. Screen layouts are driven by profiles — the CARESCAPE platform supports up to eight configurable profiles, allowing a single monitor to carry unique default settings for different care areas or patient demographics such as pediatric ICU, neuro ICU, or emergency department. Within each profile, caregivers can customize waveform fields, parameter windows, split screens, alarm limits, arrhythmia priorities, tone, and escalation behavior. These configuration tools are a key part of the platform’s value to alarm committees and biomedical administrators driving standardization across the enterprise.

Clinical Measurements and Module Support

Core clinical measurements on the B650 are acquired through either the Patient Data Module (PDM) or the Patient Side Module (PSM/PSMP), which handle ECG, impedance respiration, temperature, SpO₂, invasive pressure, and noninvasive blood pressure. Only one primary acquisition module is active at a time. Additional parameters — including CO₂ and N₂O, anesthetic agents with identification, airway O₂, spirometry, cardiac output, SvO₂, PiCCO, and neuromonitoring — are added through compatible CARESCAPE E-series plug-in modules that mount into the pivoting module frame. ECG functionality includes 3-, 5-, 6-, and 10-lead cable support, up to eight measured ECG waveforms via the Patient Side Module, diagnostic 12-lead resting ECG with 12SL and 12RL interpolated analysis, and ACI-TIPI. Arrhythmia analysis is provided by the EK-Pro algorithm with smart lead fail and four-simultaneous-lead analysis, and ST segment monitoring is available with adjustable measurement points. SpO₂ is supported with GE TruSignal/OxyTip+, Nellcor OxiMax, and Masimo SET technology through the appropriate modules and sensors, and the B650 integrates parameters such as Perfusion Index, plethysmography variability, total hemoglobin, and regional oximetry when paired with compatible modules.

Software Versions and Platform Evolution

The CARESCAPE B650 has shipped across three major software generations since its introduction, with each generation carrying hardware and feature changes that are important to understand when evaluating a unit for purchase, repair, or fleet standardization. All versions run on a hardened Linux-based operating system with a built-in firewall, password protection, encrypted data transfer, and support for customer-uploaded security certificates.

Software Version 1 is documented in GE HealthCare Technical Manual M1165897C and established the core CARESCAPE B650 platform: the 15-inch display, pivoting module frame, PDM and PSM acquisition, CARESCAPE E-series module support, EK-Pro arrhythmia analysis, 12SL and 12RL diagnostic ECG, MUSE integration, CARESCAPE Network MC and IX connectivity, and the eight-profile configuration model. It supports adult, pediatric, and neonatal profiles and is the baseline reference for the B650’s clinical feature set.

Software Version 2 is paired with the B650 VER02 hardware platform (also referenced as B650-02, OEM product code SK4) and documented in Technical Manual 2081903-001 dated September 2016. Version 2 continues support for the CARESCAPE MC and IX networks, Webmin service interface, and InSite with ExC remote service connectivity, with updated service procedures and documentation tied to the VER02 hardware baseline. GE HealthCare maintains a Software Disaster Recovery Kit (for example, v2.0.7) for Version 2 systems to support reinstallation and recovery workflows.

Software Version 3 is the current CARESCAPE generation for the B650 and is split into two primary builds. Version 3.1 corresponds to model designation MBB313 and is covered by GE documentation including the Manual USB CARESCAPE B650, SW V3.1 family (for example, part numbers 5810398 INTL, 5845415 USA, and 5860794 China) and Service Manual 2109358-002. Version 3.2 corresponds to model designation MBB323 and is covered by Service Manual 5817395-02 and the CARESCAPE BX50 manual USB family (for example, part numbers 5829987 INTL and 5829987-02 USA). Software Version 3 adds expanded platform functionality compared with earlier generations, including dedicated software packages for operating room, post-anesthesia, intensive care, neonatal, and emergency department workflows; bidirectional integration with MUSE NX for serial 12-lead comparison at the bedside; and broader specialty parameter support such as NMT, Entropy, EEG, BIS, volumetric CO₂ and O₂, PiCCO, SvO₂, cardiac output, and dual SpO₂. GE also references EK-Pro v13 with a lower QRS detection threshold intended to reduce arrhythmia alarm burden in low-signal situations such as neonatal ECG. Version 3 systems are managed through the CARESCAPE Service Interface and the Multi Monitor Manager, which allow clinical engineering to remotely configure, upgrade, and secure monitors across the enterprise.

Software upgrades across versions are handled through GE HealthCare service channels and require OEM-supplied media and qualified service personnel. Feature availability, module compatibility, and connectivity behavior vary between Version 1, Version 2, and Version 3 builds, so matching software version to facility expectations is an important step when acquiring or redeploying B650 units. IPR Medical can help identify the software version on a given monitor and advise on compatibility with existing CARESCAPE fleet and network infrastructure.

Portability and Power

The B650 is designed to support intrahospital transport as part of its intended use. When equipped with the optional internal lithium-ion monitor battery, the B650 can maintain active monitoring as a patient is moved between care areas — for example, from an operating room to a post-anesthesia care unit — without interrupting measurement or losing clinical context. The pivoting module frame allows the module stack to be repositioned for cable management during transport, and the monitor’s battery charge status is displayed on screen with dedicated symbols. In addition to the primary monitor battery, the B650 uses a separate CPU timekeeper battery — a small lithium coin cell on the CPU board — to maintain system time when the monitor is disconnected from mains power. These two batteries serve different functions and are replaced through different service procedures, as described in the GE HealthCare service documentation.

Trending, Event Surveillance, and Clinical Decision Support

The B650 captures patient data in trends, snapshots, and event lists that clinicians can review at the bedside or from the CARESCAPE Central Station. When paired with the CARESCAPE Patient Data Module, the B650 supports up to 24 hours of trending data with 36 arrhythmia events, 10 ST segments, 20 cardiac calculations, and 10 pulmonary calculations, providing a rolling clinical record that follows the patient as the PDM is moved with them. Profile settings determine default trend behavior and the data available for review during a case. The CARESCAPE platform supports clinical decision tools such as histograms, mini-trends, and serial comparison when connected to the MUSE cardiology information system, enabling clinicians to compare a current 12-lead ECG against a patient’s prior diagnostic records directly at the bedside. Alarm management is a central part of the B650’s clinical decision support surface: alarms are organized by priority with visual and audible signals, latched alarms for important physiologic events, breakthrough alarm behavior, and remote alarm management through connected stations.

Network and EMR Integration

The B650 connects to the hospital enterprise through the CARESCAPE Network, which consists of two logical networks: the MC (Monitoring Consultation) network for monitor-to-monitor clinical data, and the IX (Information Exchange) network for services such as MUSE reporting, printing, centralized management, and remote service connectivity. Integration points include CARESCAPE Central Station, CIC Pro Clinical Information Center, iCentral, CARESCAPE Gateway, Unity Network ID for external device data, and the MUSE or MUSE NX cardiology information system. HL7 support enables EMR integration at the interface layer. Remote service access is supported through GE HealthCare’s InSite with ExC, which provides secure communication between the customer site and GE’s support center. The B650 runs a hardened Linux-based operating system with a built-in firewall, and the platform supports password protection, encrypted data transfer, and customer-uploaded security certificates. Patient admission data can be entered locally, scanned from a barcode, loaded from an ADT server over the CARESCAPE Network, or entered remotely from a central station.

FDA Device Corrections and Safety Bulletins

On April 5, 2024, GE HealthCare issued an Urgent Medical Device Correction (GE HealthCare Reference #36161) affecting certain versions of the CARESCAPE B650 and B850 monitors, as well as the CARESCAPE Canvas 1000 and CARESCAPE Canvas Smart Display. For the B650, the correction applies to models MBB313 and MBB323. According to the official field safety notice, GE HealthCare became aware of the potential for loss of monitoring on affected devices if, following replacement of the CPU timekeeper battery, a mains power loss occurs during active monitoring. In the unlikely event this happens, the display or monitor may not turn back on after mains power is restored, which could result in a delay in recognizing changes in patient condition until a replacement monitor is connected. GE HealthCare reported that no injuries had been associated with this issue at the time of the notice. The FDA classified the related action as a Class 2 recall under the Medical Device Recalls Database.

The product correction involves revisions to the B650 Service Manual and Supplemental Information Manual. The updated manuals remove planned maintenance of the CPU timekeeper battery and add a required step to reload the host software after any CPU timekeeper battery replacement using the software media included with the equipment. Facilities are directed to continue using monitors that have not had the CPU timekeeper battery replaced, and to reinstall host software on any monitor where the CPU timekeeper battery has been replaced or where replacement history is unknown. GE HealthCare also advised facilities to update planned maintenance schedules to remove CPU timekeeper battery replacement and to only change the battery when the Service Monitor Error Code 0xHOST1100 is displayed.

Recommended actions: Identify B650 units at your facility by model version (MBB313 or MBB323). For any unit where the CPU timekeeper battery has been replaced, or where replacement history is unknown, reinstall the host software using the official GE HealthCare software media included with the equipment. Update facility planned maintenance schedules to remove scheduled CPU timekeeper battery replacement and only replace the battery when the Service Monitor Error Code 0xHOST1100 is displayed, followed by a host software reinstall. Download the current Service Manual and Supplemental Information Manual from the GE HealthCare Customer Documentation Portal, and destroy or delete any outdated copies of those manuals as instructed in the field safety notice. If a monitor fails to start up following a mains power loss during active monitoring, continue patient assessment with alternative methods until a replacement monitor is connected.

Because FDA device corrections and safety communications may be updated or expanded after the date of this listing, IPR Medical recommends that facilities also check the FDA’s official resources directly to ensure they have the most current information.
