Overview

Baxter InfusO.R. Smart Labels are the drug-library mechanism for the Baxter InfusO.R. anesthesia syringe pump. Each label is a magnetically coded card that snaps to the metal shield on the pump’s front panel and tells the pump’s microcomputer which intravenous agent is being delivered. Once recognized, the label reconfigures the pump’s three rotary input switches with the correct delivery units (mcg/kg/min, mcg/min, mg/min, or mL/hr), the appropriate infusion-rate range, the body-weight scaling behavior, and where applicable a bolus option scaled in mcg/kg. The label set is fixed at ten positions (L01 through L10), and most label numbers carry more than one drug because the underlying pump rate behavior is shared across agents that dose similarly. Smart Labels are essential consumables: without the correct label installed, the InfusO.R. pump cannot deliver the agent in its native clinical units, and the operator must rely on manual calculation, defeating the safety benefit the system was designed to provide. IPR Medical stocks individual Smart Labels across the published library to keep operating-room InfusO.R. fleets supportable as Baxter OEM channels have wound down.

How the Smart Label Encodes a Drug

Inside each Smart Label, high-strength magnets are placed in a specific pattern. When the label is positioned on the pump’s front panel, those magnets line up with three pairs of Hall-effect sensors mounted on a small circuit board behind the front switch panel. Each Hall pair can detect a north-pole-active magnet, a south-pole-active magnet, or no magnet — giving three possible states per sensor pair and enough combinations for the full library. The pump’s microcomputer reads the sensor outputs through the status-input shift register, decodes the magnetic pattern as a two-character label code (L01 through L10), and looks up the matching delivery profile in firmware. When the pump’s Function switch is turned to STOP/CONFIRM, the LCD displays the recognized label code so the user can verify the pump and label agree before infusion begins. If the label is missing, applied incorrectly, or unreadable, the pump alarms rather than running with a default rate profile — a deliberate failsafe in the Smart Label architecture.

Drug Library and Operating Configurations

Smart Labels operate the pump in one of three configurations depending on how the agent is clinically dosed. The first configuration is Bolus plus Infusion, used for agents commonly given as a loading or supplemental bolus followed by a continuous infusion; the top rotary switch sets the infusion rate in mcg/kg/min, the middle switch sets Body Weight, and the third switch sets the bolus in mcg/kg. Alfentanil 500 mcg/mL, Atracurium 10 mg/mL, Vecuronium 1 mg/mL, and Esmolol 10 mg/mL are representative of this configuration. The second configuration is Infusion only, body-weight scaled, used for agents that are titrated continuously and require both a high-rate and a low-rate range; only one rate switch is active at a time and the other must be zeroed, or an alarm will sound. Dopamine 4000 mcg/mL, Dobutamine 5000 mcg/mL, Nitroprusside 1000 mcg/mL, and Nitroglycerin 1000 mcg/mL operate this way. The third configuration is Infusion not based on body weight, used for agents dosed in mcg/min independent of patient size; the top switch selects a high infusion rate in mcg/min, the middle Body Weight switch is inactive, and the third switch sets a low rate. Epinephrine 20 mcg/mL and Isoproterenol 20 mcg/mL use this configuration. The full library spans L01 through L10 and is detailed in the Compatible Parts Tab.

Color Coding and Drug Class

Each Smart Label is color coded to the ASTM Drug Label Color Standard so the drug class is identifiable at a glance from across the OR. Narcotics — including alfentanil, fentanyl, sufentanil, and remifentanil — are light blue. Muscle relaxants — atracurium (Tracrium), vecuronium, rocuronium, mivacurium, and succinylcholine — are fluorescent red. Vasopressors are violet, hypotensive agents are violet with white stripes, induction agents (propofol, thiopental, midazolam) are yellow, and other agents (General mL/hr) are white. The color band runs along the top edge of the label so it remains visible even when the label is mounted to the pump face. Because the color and the underlying magnetic code are independent encodings of the same agent class, the color is a clinical cross-check on what the pump’s microcomputer has decoded — a visual confirmation that complements the LCD-displayed label code.

Pediatric Doses and Low-Concentration Variants

Several Smart Labels carry pediatric-dose or low-concentration variants of the same parent drug, with different rate scaling and body-weight ranges to suit smaller patients. Propofol 10 mg/mL has both an adult and a pediatric-dose label that share L02 but adjust the body-weight scaling. Mivacurium 2 mg/mL has a pediatric-dose variant on L01 with a lower body-weight default. Vecuronium and atracurium each carry pediatric-dose labels. Sufentanil and fentanyl each have low-concentration labels on L01 that match the lighter dilutions used for prolonged anesthesia and pediatric cases. The pediatric or low-concentration variant is identified by the printed drug name and dilution on the label itself; the magnetic code may match the adult code at the same L-number, but the rotary-switch settings printed on the label reflect the pediatric or low-concentration dosing range. Always verify the drug name and concentration printed on the label match the syringe contents before starting an infusion.

Setup, Verification, and Care

Smart Label setup is straightforward: attach the desired label to the pump face, draw up the correct drug at the dilution printed on the label, load the matching 20 cc or 60 cc B-D or Monoject syringe into the pump, set the Syringe Manufacturer Selector switch on the pump to match (Mono or B-D), and turn the Function switch to STOP/CONFIRM. The pump LCD should display the two-character label code, which the operator confirms against the visible label before turning the Function switch to INFUSE. If the displayed code does not match the printed label number or the LCD shows a missing-label alarm, the label should be reseated or replaced. Smart Labels are not waterproof and the pump itself is drip-proof only; both should be wiped with a cloth dampened with water or mild detergent and kept clear of alcohol-based cleaners, which can degrade the label substrate and the plastic case. Labels should be stored flat in a clean dry location away from strong magnetic fields when not on the pump.

OEM Support Status and Secondary-Market Availability

Baxter Healthcare announced the end-of-life for the InfusO.R. infusion pump in February 2012, with manufacturer service and parts support ending December 31, 2012. Since then, the InfusO.R. pump and its Smart Label library have remained in clinical use across many anesthesia and outpatient surgical facilities, supported entirely by the secondary market. Replacement Smart Labels — particularly the most heavily used codes such as L02 Propofol, L01 Alfentanil and Midazolam, L04 Vecuronium and Rocuronium, and L09 Nitroglycerin and Nitroprusside — are difficult to source from any single supplier because OEM new-build production is discontinued. IPR Medical maintains a working stock of individual labels recovered through equipment trade-ins, decommissioning programs, and direct purchases from facilities migrating to newer drug-library pumps. Available label codes and per-drug stock vary; the IPR Medical sales team can confirm current availability of any specific drug label by phone or email.

Service and Maintenance Considerations

Smart Labels are passive accessories — they contain no electronics, no batteries, and no firmware. Their condition is therefore primarily mechanical: the magnets must remain seated in the label and the printed face must remain legible. Bent or creased labels, labels with debris on the magnetic face, or labels whose printed drug name has worn off should be retired. On the pump side, intermittent or incorrect label recognition is most often traced to the Hall-effect sensor board behind the front switch panel rather than to the label itself; the InfusO.R. Service Manual covers Hall sensor board inspection, the front cover and hall effect assembly disassembly procedure, and verification of label decoding at the pump’s microprocessor. When a label and a known-good pump together fail to produce the correct LCD code, the label should be cross-checked against another pump before being declared defective. IPR Medical can assist with both label-side and pump-side troubleshooting and will exchange or replace labels confirmed to be unreadable on multiple known-good pumps.
