Overview
The Alaris MedSystem III is a three-channel volumetric infusion pump developed by IVAC Medical Systems and carried through the Alaris, Cardinal Health, CareFusion, and BD product histories. It is intended for facilities that deliver fluids, medications, blood, and blood products using continuous or intermittent infusion through IV, intra-arterial, subcutaneous, epidural, enteral, or fluid-space irrigation routes, and is cleared for adult, pediatric, and neonatal patients. Three independent fluid delivery channels are packaged in approximately the footprint of a single conventional pump, which is part of why the platform remained in active hospital, transport, and home-care use long after its initial clearance. The MedSystem III ships in four documented hardware variants (models 2860, 2863, 2865, and 2866) that share core delivery performance and accept the same dedicated administration sets and accessory ecosystem.
Display and User Interface
The MedSystem III uses a single-page programming interface with three dedicated channel keys (A, B, C) and a row of softkeys whose labels change to match the active page. A Standard Display page presents per-channel status — Stopped, Standby, KVO, ALARM, FAULT, or SERVICE — alongside rate, volume remaining, and volume infused for any infusing channel. Dedicated ON/OFF, STANDARD DISPLAY, MORE OPTIONS, and START/STOP keys keep core controls predictable across care areas, and the front panel supports rapid scrolling via Up/Down/Fast Up/Fast Down keys when entering rate, volume, and time values. The interface presents a Note field for clinician messages, a BatLog page used by biomedical engineering to review battery state of health, and configurable softkey prompts so departments can surface only the values they need at the bedside.
Drug Library and Dose Error Reduction
The MedSystem III ships with a Drug List Editor and the Dose Rate Calculator (DRC). The DRC calculates either a volumetric rate or a dose rate from patient weight, drug concentration (drug amount and diluent volume), and dosing parameters. Weight can only be entered in kilograms but is displayed in both kilograms and pounds, and changes to weight or concentration force the pump to recalculate the dosing regimen so settings remain internally consistent. With DRC activated, the Standard Display shows rate, dosing regimen, and drug name, which gives the clinician a single page that confirms what is being infused, how fast, and against which patient parameters. The drug list is held in non-volatile memory and is configured through Field Maintenance Software (FMS), the same biomedical-engineering tool used for service, configuration, and troubleshooting. If the lithium memory backup battery is depleted, the drug list is lost and must be reloaded via FMS, which is a useful service-planning detail for facilities running long-lived inventory.
Software Versions and Platform Evolution
The MedSystem III platform was first cleared by FDA under K961486 in 1998 to IVAC Medical Systems, with product code MRZ and regulation 21 CFR 880.5725. A later submission under K012383 (Alaris Medical Systems, 2001) covered the MedSystem III alongside the Gemini PC-1/PC-2/PC-2TX/PC-4 and Signature Edition families under product code FRN, reflecting how Alaris consolidated its pump portfolio submissions. The product carried IVAC, Alaris, Cardinal Health, Alaris Products, CareFusion, and BD branding across its commercial life as the manufacturer changed hands, but the core hardware platform and the dedicated cassette-based administration sets remained backward-compatible across the 2860, 2863, 2865, and 2866 variants. The “with Advanced Dose Rate Calculation and Drug List Editor” feature set referenced in current operator documentation reflects the mature firmware that shipped on later 2865/2866 hardware; biomed teams supporting mixed fleets should verify the firmware revision on a per-unit basis through the Field Maintenance Software, since older units in service may not have the full DRC feature set installed. IPR Medical can confirm the firmware revision on a specific serial number on request.
Battery, Mounting, and Multi-Channel Operation
The MedSystem III weighs roughly 5.1 lbs with its integrated rotating pole clamp, which makes it well suited to transport, EMS, and ambulatory workflows where a single device must cover three simultaneous therapies without occupying three pole positions. The rechargeable NiCd main battery provides a documented six-hour minimum runtime with all three channels infusing at 125 ml/hr and two minutes of backlight use per hour, and retains 80% of rated capacity after 500 charge cycles or 90% after three months of continuous AC charging. A non-rechargeable lithium memory-backup battery preserves the drug list, instrument settings, and event log when the main battery is removed or fully discharged; replacement of either battery is restricted to qualified service personnel. The pump draws 6 watts on AC power, ground continuity is specified at 0.9 ohm maximum, and leakage current at 100 microamps maximum, which keeps the unit well inside standard electrical safety test envelopes.
Alarms, Occlusion Detection, and Air-in-Line Management
The MedSystem III divides alarm conditions into advisories (infusion continues, two beeps and slow flashing red), alarms (infusion stops, four rapid beeps and rapidly flashing red), faults (numeric fault codes with continuous siren and rapid flash), and Watchdog events (blank screen, continuous tone, infusion stopped). Documented alarm categories include Battery Low, Battery Depleted, Cassette Jammed, Cassette Not Latched, Cassette Removed, Check Fluid Side, Faulty Cassette, Fluid-Side Occluded, Patient-Side Occluded, Pumping Latch Closed, and Rate/Vol Settings Cleared, with on-screen guidance for clinician response on each. Air-in-Line detection defaults to a 500 µl threshold across most Device Types and tightens to 50 µl on the Neonatal Device Type, while occlusion detection defaults to 15 psi except on the Controller Pressure Device Type, which uses 3 ft H₂O. These thresholds, along with maximum rate, maximum volume, and baseline pressure, are configurable per Device Type so a single fleet can be tuned for OR, NICU, and general medical-surgical use without changing hardware.
Service and Maintenance Considerations
The MedSystem III is supported by a dedicated Technical Service Manual that defines check-in tests for electrical safety, cassette and sensor function, patient-side and fluid-side occlusion detection, air-in-line behavior, volume accuracy, and Watchdog audio. OEM documentation recommends periodic inspections every 12 months, with check-in tests carried out on a primed cassette using clean tap water. Field Maintenance Software is the standard biomedical-engineering tool for configuration changes, drug list management, event log retrieval, and firmware service tasks; replacement of the main NiCd battery and the lithium memory backup are explicitly restricted to qualified service technicians. Because the MedSystem III predates BD’s networked Alaris System architecture, it does not connect to hospital wireless infrastructure or push infusion data into modern EMR systems automatically; biomedical and clinical engineering teams typically integrate documentation through manual charting or by using the pump’s serial communications port for clinical monitoring and configuration. IPR Medical can assist with troubleshooting, service exchanges, parts, and full pump repair.
