GE E-sCAiO CARESCAPE Respiratory Module | 5-Agent, Sidestream
The GE E-sCAiO is a single-width, small-form-factor plug-in respiratory module from GE Healthcare’s CARESCAPE Respiratory Module family — the next-generation successor to the original Compact Airway Module line (E-CAiO/E-COV). It uses sidestream sampling and GE infrared and paramagnetic sensor technology to measure inspired and end-tidal CO2, N2O, O2, and all five common anesthetic agents (halothane, enflurane, isoflurane, sevoflurane, desflurane) with automatic agent identification and two-agent mixture detection.
The “s” denotes the small footprint that frees up slot space in the CARESCAPE B650 for the Entropy module or other parameter modules. The E-sCAiO is the gas-and-agent-only variant — without patient spirometry (V) or gas exchange (X) — and is compatible with CARESCAPE B450, B650, and B850 monitors, the B40 monitor, and the Aisys CS2, Avance CS2, and Carestation 620/650/650c anesthesia workstations.

Overview
The GE E-sCAiO is the agent-monitoring entry point of GE Healthcare’s CARESCAPE Respiratory Module family — the small-form-factor successor to the original Compact Airway Module line (E-CAiO/E-COV) introduced to free up valuable slot space in the CARESCAPE B650 and B850. In one single-width module slot it provides full anesthesia gas monitoring: sidestream CO2, N2O, patient O2, and identification and quantification of all five common anesthetic agents (halothane, enflurane, isoflurane, sevoflurane, desflurane) using GE infrared technology for CO2/N2O/agents and GE differential paramagnetic technology for O2. The naming follows GE’s respiratory module nomenclature — “s” for the small footprint, “C” for CO2 and N2O, “A” for anesthetic agents, “i” for agent identification, and “O” for patient O2. The E-sCAiO does not include patient spirometry (V) or gas exchange (X), which is what distinguishes it from the E-sCAiOV and E-sCAiOVX siblings. Compared to the predecessor E-CAiO, the E-sCAiO is 60% lighter (1.5 lb vs. 3.5 lb), 50% narrower in width, draws less power, and uses a lower 120 ml/min sample flow rate (vs. 200 ml/min). It is compatible with CARESCAPE B450, B650, B850, and B40(i) monitors as well as the Aisys CS2, Avance CS2, and Carestation 620/650/650c anesthesia workstations.
Display and User Interface
The E-sCAiO itself does not include a screen. Display, alarms, trends, and configuration are managed by the host monitor or workstation. The module front panel carries the gas sample inlet, gas sample exhaust port, and the D-fend Pro water trap (which mounts and demounts with a quick-release motion for cleaning or replacement). CO2, O2, N2O, and anesthetic agent waveforms and inspired/end-tidal numerics inherit the host’s screen layout, color scheme, and care-area configuration. N2O and anesthetic agent values display only when the host is configured with anesthesia software (CARESCAPE ANE/PACU packages); on ICU or ED packages, N2O and AA values are not shown — though they are still measured internally by the module and used for CO2 and O2 cross-effect compensation. Alarm thresholds for FiCO2, EtCO2, FiO2, EtO2, FiN2O, EtAA, FiAA, and respiration rate are all set on the host. Inspired and end-tidal values update breath-by-breath, supporting the rapid-feedback workflow expected during induction and emergence.
Clinical Measurements and Gas Analysis Capability
CO2 is measured by GE infrared absorption over 0 to 15 vol% with ±(0.2 vol% + 2% of reading) accuracy and a rise time below 260 ms with 2 m and 3 m sampling lines (below 310 ms with 6 m lines). Respiration rate is calculated from the CO2 waveform over a notably wider 4 to 100 breaths/min range than the predecessor E-CAiO’s 4 to 60 — accommodating high-frequency respiration in pediatric and neonatal patients — with ±1 breath/min accuracy in the 4–20 range and ±5% in the 20–100 range. Patient O2 is measured by GE differential paramagnetic technology over 0 to 100 vol% with ±(1 vol% + 2% of reading) accuracy, returning FiO2, EtO2, FiO2-EtO2 difference, and waveform; rise time matches the CO2 channel. N2O is measured by infrared over 0 to 100 vol% with ±(2 vol% + 2% of reading) accuracy for concentrations up to 85%. Anesthetic agents are measured and identified by infrared with measurement ranges of 0 to 8 vol% for sevoflurane, 0 to 20 vol% for desflurane, and 0 to 6 vol% for isoflurane, enflurane, and halothane, all with ±(0.15 vol% + 5% of reading) accuracy. The module identifies single agents and detects two-agent mixtures with detection time below 20 seconds and a 0.15 vol% identification threshold, and provides MAC and MACage calculations on hosts that support those features. All measurements include automatic compensation for atmospheric pressure variation from 660 to 1060 mbar, temperature, and CO2/N2O and CO2/O2 collision broadening — the same compensation set used across the GE airway gas monitoring platform.
Module Family Context — When to Choose E-sCAiO vs. E-sCAiOV vs. E-sCAiOVX
Within the CARESCAPE Respiratory Module family, the trade-offs follow the predecessor Compact Airway pattern. The E-sCAiO is the gas-and-agent-only variant — full anesthesia gas monitoring but no patient spirometry and no gas exchange. The E-sCAiOV adds GE-patented D-lite, D-lite+, D-lite++, and Pedi-lite/Pedi-lite+ flow sensor support for patient spirometry: airway pressures, tidal and minute volumes, compliance, airway resistance, I:E ratio, pressure-volume and flow-volume loops (storable and recallable for comparison), and continuous intrinsic, extrinsic, and total PEEP. The E-sCAiOVX adds direct measurement of oxygen consumption (VO2) and carbon dioxide production (VCO2) over a 20 to 999 ml/min range, with derived energy expenditure (EE) and respiratory quotient (RQ) calculated by CARESCAPE B650 and B850 hosts. The non-anesthetic-agent siblings (E-sCO, E-sCOV, E-sCOVX) drop the agent measurement entirely and are intended for critical care use. Facilities standardizing on a CARESCAPE B650 or B850 fleet for OR use commonly choose the E-sCAiO for plain anesthesia rooms, the E-sCAiOV for cases requiring lung-protective ventilation strategy, and the E-sCAiOVX for ICU step-down beds where indirect calorimetry guides ventilator weaning protocols. The “E-sCAiOE” and “E-sCAiOVE” variants seen in some FDA recall scope documents are regional/configuration variants of the same family.
Software Versions and Platform Evolution
The E-sCAiO is a hardware acquisition module with measurement firmware resident on internal sensor and CPU boards. Display software, alarm logic, trend storage, and care-area-specific feature gating all run on the host monitor. On CARESCAPE B450, B650, and B850 hosts, the E-sCAiO requires ESP (Embedded Software Package) version 2.4 or later to be recognized — older CARESCAPE software baselines that fully support the predecessor E-CAiO will not display the E-sCAiO until the host is updated. The module also runs on the smaller B40(i) Patient Monitor and on the Aisys CS2, Avance CS2, and Carestation 620/650/650c anesthesia workstations, which manage the gas data through their own internal software. The displayed parameter set varies by care-area package: full N2O and AA in the ANE and PACU packages on CARESCAPE hosts, CO2 and O2 only in the ICU and ED packages (N2O and AA still measured internally for cross-effect compensation, just not displayed). Gas Exchange (X) features, where present on the VX variants, are calculated and displayed only by CARESCAPE B650 and B850. Module assembly SKUs vary by regional catalog and language configuration; commonly cataloged GE part numbers include 5697069 (E-sCAiO-00 reference) and M1199109 (alternate cataloging). IPR Medical can confirm the exact GE part number that matches a given facility’s revision and configuration requirement.
Service and Maintenance Considerations
Like all sidestream gas modules, the E-sCAiO is calibration-dependent and consumable-dependent in ways the hemodynamic modules (E-PSM, E-PSMP, PDM) are not. The D-fend Pro water trap (GE part number M1182629) is a user-replaceable consumable separating water and mucus from the gas sample line; GE specifies the D-fend Pro variant for use across the CARESCAPE Respiratory Module family — note that this is a different water trap from the original D-fend used on the predecessor E-CAiO. Sample lines are single-patient-use disposables. Periodic calibration with GE-recommended calibration gas is required at OEM-specified intervals. The internal gas sampling pump, infrared bench, and paramagnetic O2 sensor are the most failure-prone elements on aged modules. The module’s expanded functional alarm set (“Disconnected water trap,” “Partially blocked sample line or water trap,” “Low gas sampling flow,” “Blocked sample line or water trap,” “Blocked sample gas outflow”) provides more granular fault isolation than the predecessor generation, helping clinical engineering route faults to the correct service action. IPR Medical performs full functional testing, calibration with traceable gas standards, and electrical safety testing as part of refurbishment. We also confirm serial number and manufacturing date against the FDA Class I recall scope as part of intake on every E-sCAiO module we acquire.
Trending and Anesthetic Record-Keeping
Clinical data acquired by the E-sCAiO — CO2 waveform, EtCO2, FiCO2, respiration rate, FiO2, EtO2, FiN2O, EtN2O, agent identification, FiAA, EtAA, MAC, MACage — is sent over the module bus to the host, which is responsible for trend storage, alarm management, and EMR or AIMS export. On CARESCAPE B650 and B850 platforms running anesthesia software, trends are typically stored breath-by-breath for the most recent window and at decimated intervals over multi-hour periods, supporting standard anesthesia record-keeping workflows. On the Aisys CS2, Avance CS2, and Carestation 620/650/650c workstations, gas data integrates directly with the workstation’s anesthesia record. Integration with GE Centricity Anesthesia AIMS or third-party AIMS via HL7 is handled at the host level, not the module level — the same E-sCAiO module producing data on a CARESCAPE B850 in OR 1 will produce records identically on an Aisys CS2 in OR 4 if both hosts are configured to the same AIMS. Pressure-volume and flow-volume loops are not supported on the E-sCAiO (those are spirometry features handled by the V variants).
Network and EMR Integration
The E-sCAiO itself is not a networked device — it communicates only with its host over the module bus. EMR connectivity, AIMS export, and CARESCAPE Central Station communication are all managed by the host’s MC and IX network interfaces (on CARESCAPE B650/B850) or by the workstation’s internal network stack (on Aisys/Avance/Carestation). On CARESCAPE Monitor B650 and B850 hosts, gas data flows through the host’s MC Network to the CARESCAPE Central Station and through the IX Network to the hospital EMR, AIMS server, or HL7 gateway. This means an E-sCAiO module deployed in a facility’s existing CARESCAPE infrastructure inherits whatever EMR and AIMS integration the host monitor is already configured for — no separate networking work is required at the module level. The module’s small footprint and low power consumption also make it the preferred gas module for the B40(i) bedside monitor in non-OR settings where the predecessor E-CAiO would not fit.
FDA Device Corrections and Safety Bulletins
The E-sCAiO has been subject to two FDA-classified recall actions. The first and most serious is an FDA Class I recall initiated by GE Healthcare on September 27, 2019 and given Class I designation by the FDA on February 4, 2020, covering CARESCAPE Respiratory Modules manufactured from June 7, 2019 through August 16, 2019. The recall scope specifically includes the E-sCAiO (UDI 00840682104180), E-sCO (00840682104173), E-sCOVX (00840682104289), E-sCAiOV (00840682104142), E-sCAiOE, E-sCAiOVE, and the related N-CAiO Airway Gas Option (00840682104074). The root cause was an oxygen sensor manufacturing defect that could cause incorrect inspired and end-tidal O2 values, with risk of hyperoxia or hypoxia leading to potential organ damage, tissue injury, increased infection risk, or death. The U.S. recall covered 167 CARESCAPE modules and 12 N-CAiO units; no injuries or deaths were reported. GE Healthcare’s recommended action was module replacement at no cost; for facilities without an immediate replacement available, GE provided daily check instructions to verify the module remained within specification and noted that affected modules can deteriorate over time. A separate, less severe FDA Class 2 recall has also been recorded affecting the E-sCO, E-sCOV, E-sCAiO, and E-sCAiOV family for related module-family issues. The E-sCAiO is a different module family from the original Compact Airway Module E-CAiO (no “s”) which was subject to a separate, closed-out 2014 OEM field safety correction for a gas sampling pump hex screw issue.
Because FDA device corrections and safety communications may be updated or expanded after the date of this listing, IPR Medical recommends that facilities also check the FDA’s official resources directly to ensure they have the most current information.
